Schedule C - Service Provider Information

This document is the Schedule C - Service Provider Information form that is filed as an attachment to the Form 5500 Annual Report. It requires the plan to list the top 40 service providers who received compensation of $5,000 or more, provide their identifying information and relationship to the plan, and report the amount and nature of the compensation paid.
