6D04 Additional Allegations During the Perfection Stage

When additional allegations are raised during perfection, the Investigator should follow these steps:

• Obtain a written statement and attach it to/amend the original complaint form.
• If the Investigator has not issued Letter L-11 – 10-day Notice or Closure to Employer/Contractor when the complainant raises the additional allegations, the new allegations are included in the 10-day notification letter.
• If Letter L-11 – 10-day Notice or Closure to Employer/Contractor has already been issued, the Investigator must send an updated letter to the contractor to notify them of the additional allegations.

The Investigator must ensure any additional documentation is uploaded and maintained in CMS.
