Direct Beneficiary Monitoring Form

This form is a questionnaire for recording information about direct beneficiaries such as their demographic information, education history, employment history, information on the individuals debts/income/assets/expenditures, vulnerabilities to exploitation and post-training questions. The contents of this form are both in English and Nepali.
