The final two panels show all insurance carriers and self-insured employers listed alphabetically (with corresponding Act(s) covered). Acts are depicted as LS (LHWCA), DB (DBA), OC (OCSLA), NF (NAFI), DC (DCCA) under the ACTS COVERED column.
In very few cases an authorization date under an extension to the LHWCA will differ from the date shown.
