COLUMBUS, Ohio — Two workers suffered partial amputation of their left middle fingers in separate incidents at Columbus-based ITC Manufacturing because the company failed to protect them from machinery operating parts. The injuries occurred just months after the electrocution of another worker when he touched live wires at the company's facility.

PHILADELPHIA — Roofing company owner James J. McCullagh pleaded guilty today to one count of willfully violating an Occupational Safety and Health Administration regulation causing death to an employee, four counts of making false statements, and one count of obstruction of justice.

BARABOO, Wis. — A worker repainting a Baraboo water tower suffered severe injuries when he fell inside the tower to a platform below. Before falling, he had been cleaning and vacuuming debris while two painters sprayed the tower's interior with xylene-based paint.

COLONA, Ill. — Workers were overexposed to the solvent methylene chloride and other hazards while they reconditioned bucket trucks at a Colona dealership and repair facility, inspectors from the U.S. Department of Labor's Occupational Safety and Health Administration found. When workers inhale and absorb the solvent through the skin, it can harm the heart and nervous system.

SAN ANTONIO — Gabriel Palacios died after falling 35 feet from a roof while working for San Antonio-based Longhorn Contractors. The company failed to provide appropriate fall protection equipment and training, which led to his death, a federal inspection found.

MCCOMB, Ohio — A 41-year-old worker cleaning product waste from a factory floor suffered severe scalp injuries and multiple lacerations when a machine caught her hairnet, resulting in her hospitalization for four days. This is the third serious injury involving machine hazards reported this year by Hearthside Food Solutions LLC, which bakes cookies and crackers for national brands.

NEW YORK — A receptionist fired illegally at a Bronx hair salon in June 2012 for telling her colleagues about the hazards of a formaldehyde-containing straightener the salon used will be compensated for unlawful retaliation, and her former employer will take corrective action, according to a federal consent judgment obtained by the U.S. Department of Labor.

The agency found that the employee fell into an unguarded conveyer opening while breaking ice, which resulted in the amputation of both of his legs below the knee.
