Medical and Employment Documentation

Your case file must contain evidence of covered employment, a diagnosed medical condition and causation. Causation means a demonstrated relationship between the employment, exposures and the diagnosed condition.

Include Form EE-3 to verify covered employment. To help your claim, you can submit additional employment evidence including contracts, pay stubs, W2s, etc. Statements from co-workers supporting your employment during a covered time period may also be submitted. We may also attempt to verify the claimed employment with the Department of Energy, a corporate verifier, and the Social Security Administration.
