SUBJECT: Updating Social Security Administration (SSA) FAX number to send corrected Form SSA-581 (Authorization to Obtain Earnings Data from the SSA).

This Circular notifies Division of Energy Employees Occupational Illness Compensation staff that the SSA has updated their FAX number to send corrected SSA-581 forms. After the claims examiner makes the necessary corrections to a SSA-581 that SSA has previously rejected as deficient, staff is to fax the corrected SSA-581 to 410-594-2054.
