BRAINTREE, MA – Four months after citing a Quincy roofing and construction contractor – with a long history of exposing its employees to dangerous fall hazards and who reneged on a 2017 federal settlement agreement – inspectors with the U.S. Department of Labor’s Occupational Safety and Health Administration discovered the employer again knowingly exposing workers to serious injuries or worse.

JACKSON, MS – Federal investigators have determined that the employer of a 25-year-old welder – who suffered fatal injuries in an explosion at a Flora work site in July 2022 – could have prevented the tragedy by following federal workplace safety standards.

TALLAHASSEE, FL – Three construction contractors might have prevented the death of a 31-year-old laborer who suffered fatal injuries after falling while installing roof trusses at a Tallahassee work site on Aug. 17, 2022, a federal workplace safety investigation has found.
