Multiple serious safety violations found during investigation

HALLANDALE BEACH, Fla. — The U.S. Department of Labor's Occupational Safety and Health Administration has cited Waterfront Construction Inc. with 12 serious safety violations for exposing workers to falls, burns, and other hazards found, while workers installed seawalls, at 256 Three Islands Blvd in Hallandale Beach. The agency initiated the February 2014 inspection in response to a complaint and proposed a $44,100 penalty.

OSHA fines AFAM Concept $59,700 for repeat and serious violations at Chicago hair products manufacturing facilitys

CHICAGO – AFAM Concept Inc., a Chicago-based hair products manufacturer, has been cited by the U.S. Department of Labor's Occupational Safety and Health Administration for one repeat, 12 serious and two other- than-serious safety and health violations following a complaint inspection. OSHA has proposed penalties of $59,700 for exposing workers to dangerous machinery that can cause amputation, fall and respiratory hazards.

US Department of Labor's OSHA cites Exel Inc. for unsafe forklift operation at Toledo, Ohio, auto parts warehouse

TOLEDO. – Exel Inc.'s automotive parts warehouse and distribution center in Toledo, which distributes Chrysler brand parts, has been cited for one repeat and two serious safety violations involving unsafe operation of powered industrial vehicles. The U.S. Department of Labor's Occupational Safety and Health Administration initiated the inspection Feb. 12, 2014, after receiving complaints of a forklift incident at the facility. No injuries were reported. Proposed penalties total $52,500.;

CHICAGO —Miller Building Systems LLC has been cited for two willful safety violations for exposing workers to fall and overhead hazards while doing roofing work on a residential home under construction in Savoy. This is the fifth time the U.S. Department of Labor's Occupational Safety and Health Administration has cited the company for similar violations in the past five years. AKRON, Ohio — Packaging Corporation of America has been cited by the U.S. Department of Labor's Occupational Safety and Health Administration for eight — including five repeat — safety violations for failing to protect workers from amputation and other serious hazards. OSHA initiated an inspection Jan. 21, 2014, after receiving a complaint that workers were reaching in to unjam machines without turning off the machinery.

LAKE MILLS, Wis. — Fiberdome Inc. has agreed to limit employee exposure to styrene, pay a $2,000 penalty and accept a general duty clause citation under the Occupational Safety and Health Act alleging that an employee was exposed to styrene over the industry agreed-upon level. The agreement resolves all outstanding citations issued to Fiberdome in September 2013 by the U.S. Department of Labor's Occupational Safety and Health Administration.

BUFFALO, N.Y. — Tonawanda Coke Corp. and Kirchner LLC face a total of $161,100 in fines from the U.S. Department of Labor's Occupational Safety and Health Administration following an explosion that occurred Jan. 31, 2014, at the 3875 River Road plant in Tonawanda. The explosion collapsed brick walls, damaged electrical equipment and injured two permanent plant employees and one temporary employee.

CLARKSBURG, W.Va. — Following the collapse of a Clarksburg communication tower in February 2014 that seriously injured two and claimed the lives of two employees and a volunteer firefighter, S and S Communication Specialists Inc. has been cited for two serious workplace safety violations. The citations issued to the Hulbert, Oklahoma-based company follow an investigation by the U.S. Department of Labor's Occupational Safety and Health Administration.
