3. OMB Approval.OMB has approved extension of the Benefit Appeals Report, ETA 5130, information collection according to the Paperwork Reduction Act of 1995 under OMB approval No. 1205-0172 to expire November 30, 2007.

4. Burden Disclosure. Persons are not required to respond to this data collection unless it displays an OMB approval number. The state reporting burden for this collection of information is estimated to average one (1) hour per report, including the time for reviewing instructions, gathering and
maintaining the data needed, and submitting the data in the required report form. The respondent's obligation to furnish this information is mandatory (SSA, Sec. 303(a)(6)).
