SUBJECT : Annual Reporting Requirements for JTPA Title II-A/C Service Delivery Area (SDA) Programs

  1. Purpose. To transmit to States the OMB approval number and final instructions for the JTPA Annual Service Delivery Area (JASDA) report (ETA 9045).

  2. Reference. TEIN 42-93.

  3. Background. In March 1994, the proposed JASDA report format and instructions for completion of this report were provided to the States pending OMB approval. This transmittal is to confirm that the original format and these final reporting requirements have OMB approval and that States (including single SDA States) should implement this report.

  4. OMB Approval and Reporting Burden. The JASDA report requirements have been approved by the Office of Management and Budget (OMB) according to the Paperwork Reduction Act of 1980, as amended. The JASDA report has been approved under OMB Approval Number 1205-0341 and the expiration date is September 30, 1996.

  5. Reporting Requirements. The initial required submittal of the JASDA report will be for the period ended June 30, 1994. JASDA reports are due in the DOL/ETA National and appropriate Regional Offices 45 days after the end of a Program Year, i.e., by August 15.

    NOTE: At the request of OMB, language has been added to the instructions for the JASDA report, in Columns (D) and (G), to include funds transferred in the entries for availability.

    The approved final instructions for completion of the JASDA report are attached.

  6. Action Required. State liaisons are requested to distribute the attached instructions to all officials within the State who need such information to implement the JASDA report requirements.
