4. Action Required. State administrators are requested to distribute the approved form to appropriate state staff. States are requested to make ETA Forms 9042A and 9042A-1 available in all local offices to persons wishing to file a petition for Trade Adjustment Assistance beginning November 4, 2002. ETA Forms 9042A and 9042A-1 should not be available to petitioners prior to November 4, 2002.
State administrators are also requested to ensure that copies of the current petition forms, ETA Form 8560, Form 9042, and their translations, are not available to petitioners after November 3, 2002. The current petition forms should be removed from local offices and from State websites.
