Revisions to Work Opportunity Tax Credit Individual Characteristics Form

  1. Purpose. To provide notice of an error on Page1, Box 5 of the Work Opportunity Tax Credit (WOTC) Individual Characteristics Form (ETA Form9061), and an error in the instructions for Box 17 of the same form. Employment Service Program Letter (ESPL) No. 5-97, dated November 25, 1996, transmitted The Work Opportunity Tax Credit Individual Characteristics Form. This ESPL transmits a revised Individual Characteristics Form (ICF) that corrects errors in that form.

  2. Reference. ESPL No. 5-97, dated November 25, 1996.

  3. Background and Information. The ICF expedites processing of certification requests by enabling employers and their contractors to indicate the specific WOTC target group under which an individual's eligibility is sought to be established. Thus, the ICF collects information that is key to the certification process. This ESPL corrects errors in the information the ICF collects and an error in the ICF's instructions on sources that may be used to document eligibility.

  4. Changes.

    1. On Page1, Box 5 of the individual Characteristics Form (ICF), the first line that reads,"EMPLOYER STATE N0." is corrected to read,"EMPLOYMENT START DATE."

    2. On Page2, instructions for Box17, examples of Documentary Evidence or Collateral Contacts, under the caption "EMPOWERMENT ZONES/ENTERPRISE COMMUNITIES,"delete the fourth item that reads, "Signed Statement From Authorized Individual w/Specific Description of Months Benefits Were Received."

  5. Action Required. State Employment Security Agency Administrators are requested to immediately discontinue use of the ICF transmitted by ESPL 5-97, dated November 25, 1996, and immediately begin use of the ICF and instructions transmitted by this ESPL. State Employment Security Agency Administrators are also requested to provide this errata notice to appropriate staff, employers and participating agencies.
