HANOVER, Pa. — Two workers are killed every month in trench collapses. Employees of C & G Refrigeration Inc. were twice exposed to many of the hazards that lead to this alarming statistic, according to investigations by the U.S. Department of Labor's Occupational Safety and Health Administration. In April 2013, OSHA cited C & G, a Hanover plumbing, heating and air conditioning contractor, with four safety violations after a trench collapse at a work site in Hanover.

ST. LOUIS — Continental Fabricators Inc. has been cited by the U.S. Department of Labor's Occupational Safety and Health Administration for serious safety and health violations. The St. Louis-based steel fabricator, which has an injury rate higher than the national average, was issued 15 citations involving amputation, confined spaces and electrical hazards. OSHA has proposed penalties of $52,500.

KANSAS CITY, Mo. — BNSF Railway Co. has been found in violation of the Federal Railroad Safety Act by the U.S. Department of Labor's Occupational Safety and Health Administration. MANSFIELD, Ohio — Workers at the Kronis Coatings Division of Jay Industries Inc. were repeatedly exposed to amputation hazards from moving machinery parts during service and maintenance work. The U.S. Department of Labor's Occupational Safety and Health Administration has cited the company, based in Mansfield, with one repeat and four serious safety violations, which carry proposed penalties of $62,400.

OSHA News Release: Jacksonville, Florida-based ShayCore Enterprises cited by OSHA following worker being injured [10/23/2014]

JACKSONVILLE, Fla. — A 30-year-old painter was hospitalized after he received second and third degree burns when his ladder came into contact with overhead power lines. As a result, the U.S. Department of Labor's Occupational Safety and Health Administration cited ShayCore Enterprises with five safety and health violations. ShayCore Enterprises was subcontracted to paint the exterior of Total Office Solutions furniture and warehouse on Emerson Street in Jacksonville.

OSHA News Release: Ensign United States Drilling continues to expose workers to potential injury or death during oil well explosions [10/23/2014]

HOUSTON, Texas — Ensign United States Drilling (S.W.) Inc. was cited with two repeat safety violations by the U.S. Department of Labor's Occupational Safety and Health Administration for exposing workers to possible hazards during oil well explosions. The inspection was part of OSHA's Regional Emphasis Program for the Upstream Oil and Gas Well Industry. The repeat violations were cited for failing to correct deficiencies found during a 2011 investigation.

GAINESVILLE, Fla. — A 39-year-old tree trimmer was killed when the mechanical arm supporting the bucket of an aerial lift suddenly collapsed, striking the cab of the truck and ejecting the worker onto the pavement. As a result of the fatality, the U.S. Department of Labor's Occupational Safety and Health Administration cited Advanced Tree Service in Newberry for five safety violations.

CLEVELAND — A complaint inspection found workers were exposed to combustible dust, amputation and other serious hazards at Hurst Auto-Truck Electric Ltd. in Cleveland. The U.S. Department of Labor's Occupational Safety and Health Administration has cited the company for 15 serious safety and health violations after the inspection and has proposed penalties of $62,300. The plant specializes in chrome plating and powder-coating finished accessories for the automotive industry.

OSHA News Release: US Labor Department and Federal Communications Commission announce working group to prevent fatalities in telecommunications industry [10/14/2014]

WASHINGTON — Kathy Pierce expected her son, Chad Weller, to come home on March 19, 2014, at the end of his shift as a cell tower climber. But Weller, always ready with a smile for his mother, never came back. He was sent up alone to fix a communication signal on top of a water tower in the rain while wearing a harness two sizes too big — and he lost his life in a fatal fall. Chad Weller is one of 11 workers who have lost their lives in 2014 while constructing or repairing cell towers.
