TEMPLE, TX – A federal workplace safety investigation into how an employee suffered serious injuries in January 2022 at a Temple manufacturing facility operated by Carpenter Co. – one of the nation’s largest cushion manufacturers – found the company had not installed adequate machine guards or locking devices on a hot laminating machine to protect workers from unsafe contact with the machine’s operating parts.

GREENCASTLE, PA – For the second time in a year, the U.S. Department of Labor has cited a Dollar General store in Greencastle for endangering its workers’ safety and continuing the nationwide discount chain’s long history of federal workplace safety violations and penalties.

SULPHUR SPRINGS, TX – A federal workplace safety investigation found that a temporary worker suffered serious injuries on Jan. 3, 2022, after being struck by a large mold at a Texas concrete polymer manufacturing company. It was determined that the employer failed to install machine guards that would have prevented the incident.

MIDDLEBURG, FL – A U.S. Department of Labor investigation into a roof collapse that caused a 43-year-old laborer’s fatal injuries at a Middleburg work site on Dec. 13, 2021, found the employer failed to follow required safety standards that might have prevented the incident.

PEORIA, IL – Federal workplace safety inspectors investigating the fatal fall of a 50-year-old journeyman electrician at Caterpillar’s Mapleton foundry have determined that the lead contractor and a nested Caterpillar electrical contractor exposed workers to fall hazards.

ATLANTA – Federal workplace safety inspectors determined that an Atlanta recycling company exposed workers to hazardous chemicals without warning them of the risks. This is the third time since 2019 that the U.S. Department of Labor’s Occupational Safety and Health Administration cited the company for similar violations.

CANTON, OH – A workplace safety investigation into a 65-year-old worker’s fatal crushing injuries at TimkenSteel in Canton found the company failed to install guards or provide proper hand tools to prevent workers from entanglement hazards while operating a bar straightener machine.

MARS, PA – An industrial insulation contractor in Ohio entered into a settlement agreement with the U.S. Department of Labor’s Occupational Safety and Health Administration to resolve litigation prompted by the findings of a June 2020 investigation into the fatal fall of one worker and serious injuries suffered by three others after they fell from a roof and scaffold at an indoor horse-riding facility in Mars. 

LUZERNE, PA – Despite the dangers of falls – a leading cause of construction industry deaths – a Strasburg framing contractor ignored federal workplace standards and exposed workers to falls as high as 14 feet while working at a Pennsylvania Department of Transportation facility.

WASHINGTON – The U.S. Department of Labor today announced a funding opportunity for $11.7 million in Susan Harwood Training Grants to support the delivery of training and education to help workers and employers identify and prevent workplace safety and health hazards.  

ROBBINSVILLE, NJ – A U.S. Department of Labor investigation into the fatal fall of a contractor at a Robbinsville frozen food manufacturer identified a wide range of potentially fatal workplace hazards at CJ TMI Manufacturing America LLC, leading the department’s Occupational Safety and Health Administration to issue the employer citations for 36 violations and propose $368,513 in penalties.
