OSHA finds roofing contractor’s safety failures led to one worker’s death, another’s severe injuries at Florida job site

ORANGE CITY, Fla. – Had their employer inspected an aerial lift properly, it might not have malfunctioned and surged forward, crushing one worker to death and severely injuring another as they removed rain gutters at a local car dealership on Sept. 30, 2015, investigators from the U.S. Department of Labor’s Occupational Safety and Health Administration have determined.

OSHA cites lack of training after logging worker’s tragic death

VALMEYER, Ill. – Federal investigators found blunt force trauma killed a 46-year-old worker when a tree inadvertently hit him during logging operations on a remote 100-acre Mississippi River levy site in Valmeyer.

One of the nation’s most dangerous occupations, the Bureau of Labor Statistics reports 77 recorded deaths in the forestry industry in 2014.

OSHA fines five companies more than $185K for not warning workers about asbestos, not providing protection and allowing other hazards

SAN ANTONIO – The U.S. Department of Labor’s Occupational Safety and Health Administration has cited five Texas companies for failing to inform construction workers renovating a San Antonio building of the presence of dangerous asbestos, and allowing other hazards. In total, the companies now face $185,150 in fines.

Wisconsin grain handling facility exposes workers to engulfment dangers

COLUMBUS, Wis. – Two to three seconds, that is how quickly flowing grain can trap a worker in a grain storage bin. Sixty seconds later, hundreds of pounds of grain can submerge and smother them. When it happens, more than half the time workers die by suffocation.

Federal safety investigators found workers faced with this and other dangers at Duffy Grain’s Columbus facility when they visited the site in September 2015. They observed workers at risk of being engulfed in grain and entangled by operating augers as they cleaned storage bins.
