The Division of Energy Employees Occupational Illness Compensation (DEEOIC) now has a form dedicated solely to the filing of claims for consequential conditions. Beginning September 7, 2024, claimants must use the new Form EE-1A to file a claim for a consequential medical condition. The new form will help claimants clearly indicate that the condition they are claiming is related to a previously accepted condition.
Under the Energy Employees Occupational Illness Compensation Program Act (EEOICPA), a consequential illness is a separately diagnosed medical condition that a physician concludes occurred or worsened because of an illness that has already been accepted as work-related under the EEOICPA. A consequential condition can arise for any reason established as being medically linked to a previously accepted work-related illness.
Claimants can digitally sign and submit Form EE-1A via the Energy Document Portal (EDP). Documents submitted within the EDP are available to DEEOIC claims staff immediately after the document upload is complete, thus eliminating mailing delays. EEOICPA stakeholders can find more information and access the EDP by visiting the EDP website at: https://eclaimant.dol.gov.
Use of the EDP is voluntary. If claimants do not wish to electronically file a claim or upload documents into the EDP, claimants can find the EE-1A and other claim forms located at the following link: https://www.dol.gov/agencies/owcp/energy/regs/compliance/claim_forms.
Forms not submitted electronically may be mailed to the following DEEOIC Central Mail Room (CMR) address:
U.S. Department of Labor
OWCP/DEEOIC
P.O. Box 8306
London, KY 40742-8306
Claimants should include the DEEOIC Case ID number on all documents mailed to the CMR.
Claimants also have the option of visiting one of our 11 Resource Centers to have staff answer questions about the Form EE-1A or assist them in filing their claim for a consequential condition. Claimants may also reach Resource Center staff, toll-free, at 1-866-888-3322.
